Easy Steps
After you purchase five clubs on our website or from an authorized U.S. Kids Golf retailer, you're eligible to receive a 6th Club Free. Follow these simple steps below:
- Download, print and complete the 6th Club Free form.
- Submit the completed form below.
- Submit original register receipt(s) or U.S. Kids Golf Packing Slip(s).
Program Guidelines
- Purchase must be made from an authorized U.S. Kids Golf Retailer.
- Used or Outlet Golf Club are not eligible for the 6th Club Free Program.
- Collect UPC(s) for at least FIVE(5) clubs. (The UPC is the bar code sticker located on the shaft of individual clubs or on the set box for full sets).
- Regular shipping rates and applicable taxes apply.
- Faxed and mailed copies in 2025 are not accepted.
Club Combinations
- Clubs are totaled individually, but must equal five (5) clubs.
- 3-club and 4-club sets require additional purchase to equal five (5) clubs.
- For 7-club sets, retain two (2) UPCs for your next redemption opportunity!

- Ultralight & Ultralight for Women: Discount Code has a $65 value for every 5 ULTRALIGHT clubs purchased.
- Tour Series 5: Discount Code has a $70 value for every 5 TOUR SERIES clubs purchased.
- Any combination of individual Ultralight and Tour Series clubs will result in the value of Ultralight ($65 discount coupon).

Notes
- Tour Series drivers, 3-woods, and hybrids may not be fully covered.
- Please allow up to two weeks from U.S. Kids Golf receipt of items to receive discount code via email.
- The code is valid for one-time use and active for 60 days to redeem in our online store.
- The DISCOUNT CODE may be used only once (no remaining balance will be retained after purchase) and is valid for any items found under the PRODUCTS tab at www.uskidsgolf.ca.
